You have by now received a text and email with a link to a private secure folder. Please upload photos of the areas you are interested in and any documents you would like us to review. You can upload pictures directly from your camera roll into the secure folder.
We offer complimentary in-person consultations directly with Dr Farrugia. However, to ensure we can accommodate all patients, a credit card is required to secure your appointment. Your card is not charged during the booking process. If you cancel or reschedule your appointment within 48 hours of your scheduled time, or fail to attend, a $50 consultation fee will be charged to the credit card on file. Also, clients are ineligible for promotional pricing after missed consultation appointments. We value your time and ask for the same consideration in return.

We simplified our intake form so you can easily complete it on a mobile phone, tablet or computer in the privacy and comfort of your own home.
Once you click the "Consultation Form" link, we will verify your mobile phone number by sending you a code to ensure your privacy and security.
We proceed to the questionnaire where we learn about how we can help and you tell us your expectations. Estimated time: 3 minutes
We will then send you a link to both your email and phone to fill in your health history. Estimated time: 7 minutes
We will send a link to your own secure folder where you can send us photos of your areas of interest and any documents you wish to send us. You can skip sending photos if you opt for an in-person consultation instead.

Our forms and electronic platforms are HIPAA secure. Your information and photos will only be seen by our trusted staff. Once we review your information, we will contact you to review your goals and offer you a consultation with Dr Farrugia if your recommended procedure is offered at BodyLuxe. It’s as simple as that!
An in-person consultation accomplishes the same goals as a virtual consultation, with the added benefit of an in-person physical exam instead of relying on photos.
You will receive a meeting link to join at the time of your video consultation with Dr Farrugia. You can easily join on your web browser.
